Dementia Care Excellence Collaborative
The Dementia Care Excellence Collaborative is a proposed 3-year CMP-funded project designed to strengthen dementia care, improve resident outcomes, and build a more confident, educated workforce across participating skilled nursing facilities.
What participating facilities receive: one Certified Alzheimer's Disease and Dementia Care Trainer (CADDCT) trained at no cost; up to 55 Certified Dementia Practitioner (CDP) certifications for staff over the life of the project; a nationally recognized dementia care curriculum delivered by your own internal trainer; a sustainable education model that continues long after the grant ends; and support in improving dementia care quality, staff confidence, family satisfaction, and workforce retention.
Project timeline: Year 1 — trainer certification, staff training begins, initial CDP certifications. Year 2 — continued staff education and implementation of best practices. Year 3 — completion of training goals, up to 55 CDPs certified, final outcomes reporting.
At this stage, facilities are only being asked to provide a letter of support including facility name, CCN, bed count, and an authorized signature. There is no financial obligation. Note: participating facilities cannot be participating in any other CMP training projects.
What participating facilities agree to
- Designate one staff member to become a CADDCT
- Support staff participation in dementia care training
- Participate in basic project reporting and outcome measurement
- Champion dementia care excellence within the organization
